How do I sign up?

Click here to see pricing, select your benefits, and enroll. Once you have completed your enrollment our office will reach out to you to schedule an appointment. If you have any questions or would like to set up your own appointment don’t hesitate to call.

What can I expect on my first visit?

We are excited to have you as our patient! On your first visit, pay the one-time enrollment fee and first monthly payment and receive the included benefits of the plan you choose. Most plans include a comprehensive exam, x-rays, and a professional cleaning.

What should I do if I have periodontal disease?

We have a Periodontal Plan specifically designed for our patients in need. It includes the periodontal maintenance visit as well as additional discounts on specific periodontal treatments. During the initial visit we will let you know if any periodontal treatments are recommended.

What forms of Payment are accepted?

Credit and Debit cards including MasterCard, Visa, American Express, and Discover are accepted. You can also use your HSA card.

What Preventive Care is included?

Your preventive regular care includes a doctor exam, all x-rays and a professional cleaning.  These are included at no additional charge and are available based on the visit frequency you choose when you sign up for your plan.

What other discounts are included?

Your plan includes discounts on any restorative treatment that needs to be completed (fillings, crowns, root canals, etc.) or on any elective treatment that you may desire (whitening, Invisalign, implants, etc.). This save you money every time treatment is done.

How do I receive a discount on procedures?

Just schedule the treatment you need and we will automatically apply the discounts.  There is no paperwork or hassle to receive the valuable discounts.

Is there a waiting period for benefits and services?

No!  There are no limitations or waiting periods for current or future dental needs.

What is a Rollover Reward?

To encourage you to use your included preventative benefits, we will reward you with points that you can use towards any treatment that you receive. Rewards accumulate after each preventative cleaning or periodontal visit and do not expire as long as your membership remains active.

Can I combine my Rewards with other family members?

Yes! Individual rewards may be combined with other family members to maximize savings.  Take advantage of having your entire family on the plan by using everyone’s rewards on a single procedure.

How Do I Cancel My Membership?

We plan on your membership continuing on a month-to-month basis for as long as you have teeth. The total monthly amount will be billed for a minimum of 6 months and is not cancellable prior. If you need to cancel after the initial 6 months give us a call and we will be happy to discontinue your plan.
